You must provide us with an email address if you want to pay by either paypal or credit card.
NOTE: If paying by check, please include your check, payable to Ohio LongArm Quilters, for full payment with your registration form. Personal checks returned to OLAQ for any reason will incur a $25 fee.
If paying by credit card, send your registration form and we will invoice you via email. You will be directed to the PayPal website where you can securely pay online by entering your credit card information. No PayPal account is required. You must complete the payment process within 5 days from the date of the email invoice to retain your spot in the classes you register for.

IMPORTANT INFORMATION
The Registration Form must be completed and mailed to the above address. Fax, telephone and online registration are NOT available.
Please include your email address on the registration form. (This is required if you want to pay by PayPal or Credit Card.) We will send you a confirmation of your registration via email. If you do not have email, you will receive a paper confirmation via US Mail.
- Scheduling of classes will be done on a first come, first served basis. We will indicate when classes are full on the class schedule webpage. If you sign up for a class that is full, a representative of OLAQ will contact you by email or phone to discuss your options.
- Boxed lunches are available on both Friday and Saturday. They must be pre-ordered on your registration form. Cost is $9.00 each.
- Tickets for the Friday evening dinner banquet can be ordered on your registration form. Deadline for purchasing tickets is October 5, 2008. See the Banquet information on page 11 for other ways to purchase tickets.
- Registration fee is $30 per person. This fee is NOT refundable. This includes processing fees and a name tag. Those pre-enrolled will also receive a Quilting with Machines logo registration gift that will be distributed at the registration desk upon check-in at the event.
- To change a class after your registration form has been processed, contact Registrar Pat Waldron at registrar@longarmohio.com. OLAQ will permit changes through September 15, 2008. After September 15, 2008, each class change will incur a $5 fee.
- Adding classes to your schedule can be done (even at the event) at any time for no extra fee if there is space available.
- To cancel your classes after your registration form has been processed, mail a copy of your confirmation along with a written request to cancel. If this is received before August 15, 2008, you will receive a full refund of class fees. If received on or after August 15, 2008 through September 15, 2008, you will receive a 50% refund of class fees. After September 15, 2008, NO refunds will be made. However, you may sell your classes to another person registered for the event. There will be a bulletin board at Quilting with Machines where you can post a notice that you have class(es) to sell. You must notify Registrar, Pat Waldron of any classes you have sold to another person.
- The Ohio LongArm Quilters reserves the right to cancel a class if fewer than 10 students sign up. OLAQ will notify you of any canceled classes that you have already signed up for. You will have the option to select another class or be issued a refund.
