Register by Mail> Download Registration Form Please fill out the Registration Form clearly and completely. Be sure to sign the form and include your email address so you may receive your confirmation. A separate Registration Form must be used for each person registering. Personal checks returned for any reason will incur a $25 fee. Credit Card Payments: include your credit card information on your registration form. Your confirmation will be returned via email. Important InformationThe Registration Form must be completed and mailed to the above address. Fax and telephone are NOT available. Please include your email address on the registration form. We will send you a confirmation of your registration via email. If you do not have email, you will receive a paper confirmation via US Mail. Scheduling of classes will be done on a first come, first served basis. We will indicate when classes are full on the class schedule webpage. If you sign up for a class that is full, the QwM registrar will contact you by email or phone to discuss your options. Boxed lunches are available on Wednesday, Thursday, Friday and Saturday. They must be pre-ordered on your registration form. Cost is $12.00 each. See page 6 of the registration form. Tickets for the Friday evening dinner banquet can be ordered on your registration form. Be sure to select your dinner choice from page 7. Deadline for purchasing tickets is September 3, 2010. See the Banquet information on page 7 forother ways to purchase tickets. Class Changes To make schedule changes after your registration form has been processed, contact Registrar Janice Kiser at ewetree53@windstream.net. Added Classes Class additions will be accepted and processed through September 3, 2010. After that date any additions will not be processed until arrival at the conference center on September 22, 2010. Dropped Classes Registered students may drop selected classes until August 20, 2010. No classes will be dropped after August 20, 2010. Refunds are subject to the criteria defined below. Students may attempt to sell their purchased classes at the show. Drop/Add Classes Students may drop/add a class through August 20, 2010 without incurring an additional fee. Drop/Add adjustments will be accepted through September 3, 2010, but a $5 fee will be assessed for each drop/add transaction. After September 3, 2010 no changes will be processed until arrival at the convention center. Refunds Refunds will be processed as follows for cancellations received: To cancel your classes mail a copy of your confirmation along with a written request to cancel to Quilting with Machines, c/o Janice Kiser, Registrar, 61 Geoppert Road, Peninsula, OH 44264 Classes may be sold to another person at QwM for the original purchase cost only (no discounted sales). All sales must be reported to the registration desk so that class rosters may be adjusted. There will be a bulletin board near QwM check in for the purpose of posting classes for sale. Quilting with Machines, Inc. reserves the right to cancel a class if class minimums are not met. QwM will notify you of any canceled classes that you have already signed up for. You will have the option to select another class or be issued a refund. |
